Your first visit to the orthodontist
Anyone can book directly with an Orthodontist – you don’t need a referral. Your dentist can also provide you with a referral.
To assist us with your care and to avoid administration delays in booking your appointment, we encourage you to fill out our Patient Questionnaire and bring it along to your consultation.

What happens during my first visit?
Your Orthodontist will listen to your concerns and undertake a detailed clinical examination. If you already have an appointment arranged but have not received any forms, please complete our New Patient Questionnaire. Orthodontic records may be taken to assist in the clinical examination or for analysing/planning the course of necessary orthodontic care.

Consultations at Macquarie Street and Mosman
Prior to your appointment, we will send you a questionnaire and information about our practice along with a reply-paid envelope. Please fill out the questionnaire and mail it back to us. One week prior to your appointment, we will call you to confirm. X-ray facilities are on-hand if required.
Consultations in Bowral, Orange and Goulburn
X-rays must be taken prior to the appointment at a local facility. We will provide you with an X-ray referral form. Prior to your appointment, we will send you a questionnaire, X-ray referral form and information on our practice and orthodontic treatments.
What are diagnostic records?
Diagnostic records include photographs, X-rays and models made of the teeth. Facial photographs are used to assess and record facial proportions, aesthetics, and smile balance. The X-rays are used to screen for problems, assess relationships of teeth and bones, and to plan your treatment. The models of the teeth are used to assess how the teeth relate to each other and record the original problem. Full records enable your orthodontist to develop your optimal and individualised plan, to maximise facial and dental aesthetics, function, and dental health.
